New Patient Registrations
The practice list is open, and we are happy to accept new patients moving into our practice area. We aim to provide caring and efficient services for our patients. Speak to a member of our reception staff if you have any questions about the practice or registration process.
How to Register
To register as a new patient at the practice. Please follow the registration process below.
- Check you live within the catchment area.
Enter your postcode to check you live within the practice boundary’ - Complete a registration form (GMS1) and new patient questionnaire.
Catchment Area
To register, you can complete the GMS1 form online. All patients will also need to complete the New Patient Questionnaire. Online registrations can only be accepted when both forms have been received. If you register using the online forms, you will have to bring Identification into the surgery prior to online access being activated.
Alternatively, you can visit reception for the forms. Please bring photo ID and address ID when registering. If you do not have the required documents, please speak to a member of the reception team.
