The practice list is open, and we are happy to accept new patients moving into our practice area. We aim to provide caring and efficient services for our patients. Speak to a member of our reception staff if you have any questions about the practice or registration process.

How to register

We are using a new NHS online service called Register with a GP Surgery that makes it easy to register with our practice.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Click on the link below to complete the online registration form and submit your request. Please read the guidance provided before you start, the form will take around 10 minutes to complete.

Paper Registration Forms

If you do not have online access or wish to register in person paper forms are available at reception.

New Patient Health Check

When your registration has been accepted you may be asked to make an appointment for a new patient health check. This helps us ensure we have accurate and up to date information about your health, pre-existing conditions and any ongoing treatment and regular medications.

This appointment will take around 20 minutes and will include:

  • Measuring your height and weight
  • A blood pressure test

You will also be asked some questions about your health and lifestyle including:

  • Whether any of your close relatives have had any medical conditions
  • If you smoke, and how much
  • If you drink alcohol, and how much
  • How much physical activity you do

Your age, gender and ethnicity will also be recorded.

If you do not have any pre-existing conditions and you are aged 40 to 74 a blood test will also be included.
